What Documents Families May Need for Benefits

Gathering the right documents before you apply for child benefits saves time and avoids delays. This guide outlines what is typically required for Poland's 800+ programme.

Standard Documents for an 800+ Application

For most straightforward applications, you will need:

Identity Documents

  • Polish ID card (dowód osobisty) or passport for the applying parent/guardian
  • If you are a foreign national: your residence card (karta pobytu) or other proof of legal residence in Poland

Child's Documents

  • Child's birth certificate — this proves the child's existence, age and parentage
  • Child's PESEL number — assigned at birth or registration in Poland. If your child was born abroad and does not yet have a PESEL, you will need to register them with the local commune (gmina) first.

Bank Account Details

  • Polish bank account number (in IBAN format) for payment
  • The account should be in the name of the applying parent/guardian

Custody Documents (if applicable)

  • If you are a single parent applying, a court order or agreement confirming custody arrangements may be required, particularly if the other parent could also be applying

For Guardians (Non-Parents)

  • Court order establishing legal guardianship
  • Identity documents as above

Additional Documents for Specific Situations

Disability Supplements

If applying for any supplement related to a child's disability, you will need:

  • Official disability certificate (orzeczenie o niepełnosprawności) issued by a local disability assessment team (Powiatowy Zespół ds. Orzekania o Niepełnosprawności)

EU Citizens / Cross-Border Situations

If the family has ties to another EU country (for example, one parent works in Germany while the other lives in Poland), the EU coordination rules apply. ZUS may request:

  • Proof of employment or residence abroad
  • Documentation from the other country's benefit authority

How to Submit Documents

  • Online through zus.pl (PUE portal) or empatia.mrips.gov.pl — scan and upload
  • By post — send copies to the relevant ZUS branch
  • In person — bring originals and copies to your ZUS branch or MOPS/GOPS office

Retain copies of everything you submit. If submitting by post, use registered mail and keep the confirmation of postage.
